This Privacy Notice is intended to comply with the Health Insurance Portability and Accountability Act of 1996, as amended (“HIPAA”). Welz will maintain the privacy of your Protected Health Information (“PHI”) as required by HIPAA and the regulations promulgated under that Act. Welz may modify this Notice from time to time in Welz sole discretion. Welz will provide notice by, at a minimum, updating this posting. You are expected to carefully review this Mobile Site Privacy Policy from time to time so you are aware of any changes. Welz collects information you share through the Site. Welz may also collect information concerning you, including, but not limited to, records and copies of your correspondence, if you contact us. When you access and use the Site, Welz may use technology to automatically collect:
• Usage Details. When you access and use the Site, Welz may automatically collect certain details of your access to and use of the Site, location data, logs, in-Site events tied to a specific user profile (including certain PII), and other communication data and the resources that you access and use on or through the Site.
• Device Information. Welz may collect information about your computer device or mobile device, if used to access the Site, and internet connection, including the device's unique device identifier, IP address, operating system, and mobile network information, if applicable.

Welz may use Google Analytics or other similar technologies (the “Analytics Technologies”) to collect and process certain information about you and your use of the Site in accordance with the terms and conditions of this Privacy Notice. The information collected by the Analytics Technologies is statistical data and does not include PII, except as set forth above, and Welz does not maintain it or associate it with PII Welz collects in other ways or receive from third parties. Welz's webserver and/or the Analytics Technologies may collect and store the following general information about you: the originating name of the domain from which you access the Internet; the date and time you access the Site; the pages of the Site you visit; the internet address of the website from which you linked directly to use; and the type of device, device ID and web browser you use to access the Site. This information is collected automatically. It is used to help Welz improve the Site by tabulating the number of visitors to the Site in terms of new users, origin of referrers, popularity of functions, amount of use, types of errors, and to make the Site more useful to you. Welz webserver and/or the Analytics Technologies may also collect Internet protocol addresses (the Internet address of a device) to track a user's session while the user remains anonymous. Welz analyze this data for certain trends in statistics, such as which parts of the Site users are using and how long they spend there.

Welz may also use “cookies” (or mobile cookies) and “web beacons.” Cookies are small files stored on your device that are used to store and track personal information. Cookies are used to remind us of who you are in order to deliver you better service. It may be possible to refuse to accept mobile cookies by activating the appropriate setting on your device. However, if you select this setting, you may be unable to access certain parts of Welz Site. Web beacons are small electronic files (also referred to as clear gifs, pixel tags, and single-pixel gifs) that permit us, for example, to count users who have visited certain pages and for other related Site statistics(for example, recording the popularity of certain Site content and verifying system and server integrity).

If collected, Welz may use location information Welz collects to provide you with the nearestlocations of healthcare clinics, healthcare providers, or other organizations that you may belooking for or that may be able to assist you.

Welz may also use these automatic data collection technologies to collect information about your online activities over time and across third-party websites, apps, or other online services (“Behavioral Tracking”). “Do Not Track” is a proposed standard that is currently under development. As it is not yet finalized, Welz adheres to the standards set out in this Notice and Welz does not currently respond to “Do Not Track” signals on the Site or on third-party websites or online services where Welz may collect information. The information Welz collects automatically is statistical data and does not include PII, except as otherwise set forth in this Mobile Site Privacy Policy, and Welz does not maintain it or associate it with PII Welz collects in other ways or receive from third parties.

Some content, including advertisements, on the Site are served by third-parties, including advertisers, ad networks and servers, content providers, your mobile device manufacturer, and your mobile service provider. Welz does not rent or sell your information, including your PII or PHI, to outside marketers. Welz may share your information (as described in this Privacy Notice) with its affiliates,subsidiaries, or divisions and with their respective officers, directors, employees, accountants, attorneys, and agents, or with Welz's business partners. Unfortunately, the transmission of information via the Internet is not completely secure. Although Welz do Welz best to protect your PII, Welz cannot guarantee the security of your PII transmitted through the Site. Any transmission of PII is at your own risk. Welz are not responsible for your circumvention of any privacy settings or security measures Welz provides.

This Notice is applicable only to the Site and does not apply to any third-party websites or to other companies or persons that may access the Site, with or without Welz permission. The Site may contain links to, and media and other content from, other websites. In addition, such third party media and content may also contain external links. The Site is not intended for children under 13 years of age. Welz does not knowingly collect PII from children under 13 through the Site. If Welz learns thae Welz has collected or received PII from a child under 13 without verification of parental consent, Welz will delete that information. Upon your request, and subject to your providing the necessary written authorizations as required by law, Welz will provide to you the PII under its control, if any, as well as information about the ways in which that information is being used and disclosed. Welz only collects PII for the purposes of providing you with services that you requested from us and the support of those services. If you demonstrate that your PII is inaccurate or incomplete, the information will be amended as appropriate. Welz will use commercially reasonable efforts to respond to any request and update your PII, if necessary.

California Civil Code Section § 1798.83 permits users of the Site that are California residents to request certain information regarding Welz’s disclosure of PII to third parties for their direct marketing purposes. This Notice and any disputes related thereto shall be governed by and construed in accordance with the laws of the State of New York, exclusive of its choice of law rules, whose courts shall have exclusive jurisdiction over all disputes arising in connection with this Notice, and the place of performance of this Privacy Notice is agreed by you to be the State of New York.
